The New York State Department of Health will be presenting a webinar On February 28, 2012 entitled "Tools for Implementation of Health Homes." The discussion will include: an explanation of the roles of designated Health Homes and Managed Care Plans, the steps that need to be taken to implement the new program, an overview of model contract language, how to bill and receive payment for services, the care management and quality metrics used for reporting; and a self assessment tool that will be used for evaluation.
